Faculty and staff have four days left to obtain an onsite biometric screening this fall through the faculty and staff wellness program.
Walk-in visits are available Nov. 23, 24, 25 and 30 from 7 a.m. to 1 p.m. in the Frances Payne Bolton School of Nursing’s Faculty Staff Lounge. Fasting for nine hours prior to the screening is necessary.
Completion of the Biometric Screening, and (HCM – Main Menu – Self Service – Benefits – Tobacco Attestation Form) are required by Nov. 30. Election of one of the university’s medical insurance plans is also necessary to receive the $25 monthly Wellness Premium Incentive in 2016. 2015 Wellness Premium Incentive recipients also have until Nov. 30 to complete one or two of the Program Incentive forms within HCM (Main Menu - Self Service - Benefits - Wellness Program Participation) to attest to their eligibility for those incentives ($100 for one program incentive or $200 for two).
